Submitting a claim with special patient information

If, while adding a claim or pre-treatment estimate, special details, such as an overage dependent or an accident resulting in needed dental care, need to be added to the patient information:

Click the Claim Details tab at the top of the claim. The Claim Details screen appears.

Patient Account Number — enter your own account number for the patient; this is for your reference only and is not used in processing.
Overage Dependent — if the patient is a dependent and over the child age limit, select the correct type of overage dependent (see Viewing member eligibility and benefits).
Assignment of Benefits — if assignment of benefits is being refused and the claim payment is to be sent to the subscriber, select No.
Patient Signature — if you have the patient’s signature assigning benefits for the claim, select Yes.
Accident — if the treatment is required because of an accident that may be covered by another form of insurance, select the correct type of accident (automobile, employment, other, or none. The default is none).

When finished, click the Return to Treatment Details to submit this claimlink, and continue adding the claim as described in Adding a claim or pre-treatment estimate.
